In the current age of hyper-modernized, nanotech and disruptive technologies there is a growing shift to focus around pain-free procedures to save the natural dentition. Even today most clinicians struggle with providing consistent profound endodontic anesthesia. Understanding the basic concepts at the deeper level and the possible interactions will not only help the clinician to provide profound anesthesia but also help the researcher to develop further technique and strategies for next generation dentistry. This lecture reviews the pain pathways and anesthetic concepts and their role in pain-free endodontics today.
